Once you have installed Form Publisher on your account and created your Google Form, you will have to open Form Publisher. This article will explain how to start with Form Publisher.
From your Google Form, click on the add-on icon at the top right of your Google Form. Then, click Form Publisher.
Click Get Started.
As your Google Form has been configured, you see the following sidebar.
From this point, you can directly read this article: '[Step 3] Select your template'.
Note If you have opened Form Publisher from an entirely new Google Form, you will see the following sidebar.
If you never used Form Publisher before and want to try with a demo, then click Try demo. Read the following article to see a quick demo to help you get started with Form Publisher.
However, if you want to configure Form Publisher by yourself, click Start setup.

Hello there.
In reference to step, I don't see a get started when I click on Form Publisher in Google Forms.
The same occurs for me, where there isn't a selection option. This is happening in several of my other add-ins as well. Perhaps Google has changed something in permissions?
Vakosile The owner of the document must be signed in as the "master" sign in on google in order to properly use the plugin.

If you are facing the issue where only the "Help" button is displayed then the workaround to follow is quite simple. Sign off your Google account, log in again, and wait a few seconds before opening the Form Publisher menu. All the other options should appear.

Hello All,
So, I worked with our IT staff to get this fixed. Here's the issue. Our IT department 'White-listed' the app for individual installation. However, it didn't work unless the FormPublisher was domain installed. Afterward, that worked.
