With Form Publisher, you can generate multiple documents every time someone submits your form.
We prepared a Google form template named Restricted Areas - Access request form.
Following the steps in the sections below, you can configure Form Publisher to generate the following documents every time respondents submit an answer.
- Form Publisher generates a Permission request PDF document and sends it to the form owner
- Form Publisher generates a Permit to visit PDF document and sends it to the respondent and the form owner
1. Copy the predefined templates
Prepare the templates needed to configure Form Publisher to generate both documents after each submission.
Procedure
Copy the pre-defined templates below, by opening them and clicking the Use Template button:
- Google form template: Restricted Areas - Access request form.
The Collect email addresses option is enabled in the Google form to send the Permit to visit document to the respondent. - Google document template: Permission request.
- Google document template: Permit to visit protected areas.
Result
All templates are now saved in your Google Drive.

2. Configure Form Publisher on the main form
Configure Form Publisher on the Google form to send a permission request document to the form owner each time a respondent submits the form.
Procedure
- Start Form Publisher.
- Click the Add-ons icon in the Google form and select Form Publisher.
- In the pop-up menu, click Get started.
- Click the Add-ons icon in the Google form and select Form Publisher.
- Configure Form Publisher to use the Permission request Google document template.
- Click Select template.
- Find the Permission request template in the Google Picker window, and click Select.
Wait for Form Publisher to finish the configuration and update the template screen.
A summary of the default settings appears.
- Click Select template.
Result
Form Publisher is now configured to generate new documents with the Permission request template. Also, Form Publisher will send the generated documents to the form owner.

Create a connected form to allow Form Publisher to use a second template.
3. Create the connected form
You create a connected form with Form Publisher anytime you need to use an additional template. For more information, see Generating multiple types of documents from the same form.
In this case, the condition is to generate both templates at once when someone submits the form.
With Form Publisher still open on the main form, you can create the connected form from the Additional templates screen.
Procedure
- Click the Change settings button and select Additional templates.
You can now define the condition based on your multiple-choice question and set up the additional template. - Click Get Started.
Form Publisher creates a copy of the Google form (named Connected to [TEMPLATE] Application for permission access).
Result
The title of the connected form and a link to open it are now available in the Additional templates screen.
Under Rules, the Additional template screen also includes options to configure the rules and conditions of when Form Publisher uses the first template to generate documents.

Configure the condition to define when Form Publisher will use the first template.
4. Configure the condition for the first template
On the main form, configure the condition to define that Form Publisher will generate both documents every time someone submits the form.
Procedure
- Under Rules, select Always create both templates option to configure Form Publisher to generate documents using both templates every time someone submits the form.
- Click Save.
Result
Form Publisher is now configured to always generate multiple documents at once. The first template configured in the main form will be generated every time someone submits it.
This also means that you do not need to define a condition for the second template because Form Publisher will use it whenever the condition for the first template is not true.

Open the connected form and configure Form Publisher with the second template.
5. Configure Form Publisher on the connected form
Configure Form Publisher on the connected form to send a Permit to visit document to the form owner and to the respondent, each time a respondent submits the form.
Procedure
- Open the connected form.
- Open and configure Form Publisher to use the Permission request Google template document.
- Click Select template.
- Find the Permit to visit protected areas template document in the Google Picker window, and click Select.
Wait for Form Publisher to finish the configuration and update the template screen.
The summary of the setup configuration by default appears.
- Click Select template.
- Click Change settings and select Sharing options and notifications.
By default, the email address of the form owner is added to the Sharing list. - Select the Notify Form respondent option to add the respondent to the Sharing list.
- Click Save to save the configuration.
Result
Form Publisher is now configured with the second template on the connected form.
As a result, each time respondents submit the form:
- The Permission request PDF document is sent to the form owner.
- The Permit to visit restricted areas PDF document is sent to the form owner and the respondent.
(Optional) Configure three or more templates
You can add a third template by creating a second connected form. To do so, use the steps in 3. Create a connected form and 4. Configure Form Publisher on the connected form.
Such configurations are helpful when you want to generate 3 different documents.
Configuring more than three templates can result in issues that are challenging to troubleshoot. As a best practice, do not use more than two templates and, when necessary, use three at most.
