Please read this article to know how to reset Form Publisher on your Google Form.
warning Resetting your Form Publisher settings is irreversible. Do that only if you are sure that you no longer want to use Form Publisher on your Google Form (or if you want to configure everything again from scratch).
1. Open Form Publisher Status
Click the add-ons icon, then Form Publisher.
Then, Form Publisher's window appears, click Status & quotas.
2. Reset Form Publisher settings
The Status sidebar appears, click Reset settings under More options then click on the Reset settings button. Click Reset to confirm.

If you open Form Publisher again, you can see that all your settings menus have been erased, and replaced by Get Started (exactly as if it was the first time you configure Form Publisher on your Form).
