You purchased a Form Publisher plan, and you don't remember if you paid by debit or credit card via our payment provider Stripe or Paypal. You can find this information in the Form Publisher add-on.

Procedure

1. Click Add-ons and select Form Publisher.
   
   
   
2. In the pop-up menu, click Launch Form Publisher.
   
   The Form Publisher for Google Forms add-on starts in the lower-right part of the screen and opens the Template and markers page.
3. Click the Menu button.
   
   The Form Publisher menu opens.
4. Select Form status and account info.
   
   
   
5. Select the Account tab.
   

Result

Under Paid with, if you see the Stripe icon, it means you purchased your plan via Stripe.

If you see the Paypal icon, it means you purchased your plan via Paypal.
