After you configure Form Publisher on a new form and Form Publisher starts to generate documents with the answers from each form response, Form Publisher names each generated file in Google Drive with the title of the template document followed by the sequential number of the form submission, also known as increment:
<<Template Title>> <<Increment>>.
In the naming convention, you can add any of the standard markers, you can add question markers (so that documents are named with the responses people provide to a question), or you can also add any text or numbers that you want to appear in each file name.
Markers are terms surrounded with double angle brackets, such as <<Template Title>>.
In the default naming convention, the term <<Template Title>> refers to the file name of the configured template.
Also, the default naming convention <<Increment>> refers to the sequential number of the submitted form response. For example, the first form response has an increment number 1, the second is 2, and so on.
To sum up, with the default naming convention <<Template Title>> <<Increment>>, the generated document from the first form response has the file name Form Publisher Template 1, the second form response generates a document with file name Form Publisher Template 2, and so on.
You can change the default naming convention to whatever meets your requirements.
